Narrative:
As first officer I was, at the time, PF. As captain was PNF, he was doing the radios. Controller asked aircraft to report OM. PNF/captain failed to report OM or get a clearance to land. This was discovered when controller questioned aircraft on the ground. Hindsight: when flying with a 'flake' captain, I should make a greater effort to cover both roles.
