Narrative:
Mar/thu/95 departed mia for mlb and the overnight of our trip at XA30. We left the crew lounge at approximately XA00 and were unaware of commercial revision #12. I had just checked my company e-mail and my mailbox and both had no reference to revision #12. On the morning of mar/fri/95, while trying to depart mlb, we could not accept a clearance that had mia STAR heat 4 (we both had heat 3). ZMA said that was not a problem, that they would work with us to get to mia. Flight was uneventful. Upon arrival at mia and receiving #12, we found that several charts in this revision affected our flight, and were out of date. The revision was delivered to the company on mar/fri/95 and they failed to inform us, or deliver to us this revision.

Title: FLC TOOK OFF WITH THEIR COMMERCIAL APCH PLATE MANUALS OUT OF DATE.
Narrative: MAR/THU/95 DEPARTED MIA FOR MLB AND THE OVERNIGHT OF OUR TRIP AT XA30. WE LEFT THE CREW LOUNGE AT APPROX XA00 AND WERE UNAWARE OF COMMERCIAL REVISION #12. UPON ARR AT MIA AND RECEIVING #12, WE FOUND THAT SEVERAL CHARTS IN THIS REVISION AFFECTED OUR FLT, AND WERE OUT OF DATE. THE REVISION WAS DELIVERED TO THE COMPANY ON MAR/FRI/95 AND THEY FAILED TO INFORM US, OR DELIVER TO US THIS REVISION.
