Narrative:
On apr/mon/95 I was the captain on a series of 3 flts for my regional airline. During the first flight from dfw to lbb, a woman passenger had difficulty breathing due to an asthma problem and asked the flight attendant if she could use our on- board oxygen bottle. The flight attendant not only let her use bottle #1, but allowed her to use bottle #2 and take it with her off the airplane when we arrived in lubbock. I was not aware of any of this until we were 1/2 way back on our return to dfw. Although we were not legal for that flight there was nothing I could do until our arrival at dfw. While we were unloading bags at lbb, one of the rampers noticed our fire extinguisher in the rear cabin area needed to be svced at once and told the flight attendant. However, she did not tell me about it until the next morning after we had flown lbb-dfw and dfw-tul making 2 illegal flts. I knew nothing about it until it was too late. The oxygen bottles were recharged/replaced in dfw, but a mechanic had to drive from dfw to tul and bring a new fire extinguisher before we could depart legally the next morning. If the flight attendants had followed proper procedures and told me about these incidents as soon as they happened, some passenger would have been inconvenienced due to the delays in recharging/replacing the effected equipment, but we would not have operated the flts illegally.

Title: ACFT EQUIP PROB. THE CABIN ATTENDANTS ALLOWED A PAX TO USE BOTH OXYGEN BOTTLES AND TO TAKE ONE OF THEM AWAY. THEY ALSO KNEW OF A FIRE EXTINGUISHER THAT NEEDED SVC AND DID NOT INFORM THE FLC UNTIL THEY HAD ALREADY FLOWN ONE OR MORE FLTS.
Narrative: ON APR/MON/95 I WAS THE CAPT ON A SERIES OF 3 FLTS FOR MY REGIONAL AIRLINE.
