Primary Problem | ATC Human Performance |
Narrative:
Executing the missed approach instructions of 'turn left to heading 070 degree' (from heading 171 degrees), 'climb and maintain 3000 ft (from 960 ft). Had nearly completed turn to 070 degrees, aircraft gear retracted/flaps retracted, climbing at approximately 900 FPM, when we passed in front of dehaviland beaver bearing 090 degrees relative same altitude. No evasive action required or made by either aircraft. Problem obscured because missed approach instructions were incorrect for the amount of traffic. The safety pilot detected the problem, since PIC was under hood performing instrument competency check. The problem may have started earlier than at the time of executing the missed approach instructions in that: ATC did not issue instructions for PIC to contact olm at IAF (haber), thus PIC initiated request with ATC to go to tower, and authority/authorized to do so about 3 mi from VOR. Thus tower may have become aware of the conflict late. Based on safety pilot observations, the turn was initiated 30 seconds before tower ordered the turn, or situation would have been worse.
Original NASA ASRS Text
Title: NMAC IN TFC PATTERN ATA DURING MISSED APCH PROC INSTRUCTIONS FROM TWR.
Narrative: EXECUTING THE MISSED APCH INSTRUCTIONS OF 'TURN L TO HDG 070 DEG' (FROM HDG 171 DEGS), 'CLB AND MAINTAIN 3000 FT (FROM 960 FT). HAD NEARLY COMPLETED TURN TO 070 DEGS, ACFT GEAR RETRACTED/FLAPS RETRACTED, CLBING AT APPROX 900 FPM, WHEN WE PASSED IN FRONT OF DEHAVILAND BEAVER BEARING 090 DEGS RELATIVE SAME ALT. NO EVASIVE ACTION REQUIRED OR MADE BY EITHER ACFT. PROB OBSCURED BECAUSE MISSED APCH INSTRUCTIONS WERE INCORRECT FOR THE AMOUNT OF TFC. THE SAFETY PLT DETECTED THE PROB, SINCE PIC WAS UNDER HOOD PERFORMING INST COMPETENCY CHK. THE PROB MAY HAVE STARTED EARLIER THAN AT THE TIME OF EXECUTING THE MISSED APCH INSTRUCTIONS IN THAT: ATC DID NOT ISSUE INSTRUCTIONS FOR PIC TO CONTACT OLM AT IAF (HABER), THUS PIC INITIATED REQUEST WITH ATC TO GO TO TWR, AND AUTH TO DO SO ABOUT 3 MI FROM VOR. THUS TWR MAY HAVE BECOME AWARE OF THE CONFLICT LATE. BASED ON SAFETY PLT OBSERVATIONS, THE TURN WAS INITIATED 30 SECONDS BEFORE TWR ORDERED THE TURN, OR SIT WOULD HAVE BEEN WORSE.
