Narrative:

No APU, had to start engine at gate, ground crew slow in ground equipment, slow push back, set brake, driver disconnected tow bar and I cleared him off headset. Set cockpit up for single engine taxi, gave marshaller signal for taxi, no acknowledgement so did it again. Marshaller didn't enforce hold signal but dropped wands momentarily. Released brakes rolled forward and hit towbar. Tug had disconnected headset but had not left aircraft and was still parked under nose. Marshaller needs to be more demanding. Driver should disconnect and be gone he said he was waiting for other traffic. Make sure you see driver move off in distance not just forward and stay under aircraft. Things may be slow but don't rush for on-time scheduling.
