Narrative:
Hazmat paperwork. On arrival at dfw, I discovered that a battery pwred wheel chair had been loaded in the forward cargo compartment. The proper form was taped to the battery signed by X in the prepared by box, and Y in checked by box. No signature was in the loaded box. No notification was on the load closeout that hazmat was on board.

Title: HAZARDOUS MATERIAL IMPROPERLY DOCUMENTED.
Narrative: HAZMAT PAPERWORK. ON ARR AT DFW, I DISCOVERED THAT A BATTERY PWRED WHEEL CHAIR HAD BEEN LOADED IN THE FORWARD CARGO COMPARTMENT. THE PROPER FORM WAS TAPED TO THE BATTERY SIGNED BY X IN THE PREPARED BY BOX, AND Y IN CHKED BY BOX. NO SIGNATURE WAS IN THE LOADED BOX. NO NOTIFICATION WAS ON THE LOAD CLOSEOUT THAT HAZMAT WAS ON BOARD.
