Narrative:

X on a training flight in a beech duchess. I was departing the san carlos airport using runway 30 and using a belmont slough departure. While at approximately 700 ft MSL another aircraft, cherokee Y, passed near and to my right traveling in the opposite direction. Y was observed to take evasive action, and that is when we spotted it. I believe the cause of this event was two fold. First, pilot error on the part of the pilot of Y upon entering the san carlos traffic pattern and lack of good judgement. Second, ATC failing to notice that this aircraft was entering the traffic area at the departure end of the active runway! Note: I recognized the aircraft in question and I could also read the n-number as it passed off my wing.

Title: SMA X HAD NMAC WITH SMA Y IN TFC PATTERN CLASS D AIRSPACE. SEE AND AVOID CONCEPT.

Narrative: X ON A TRAINING FLT IN A BEECH DUCHESS. I WAS DEPARTING THE SAN CARLOS ARPT USING RWY 30 AND USING A BELMONT SLOUGH DEP. WHILE AT APPROX 700 FT MSL ANOTHER ACFT, CHEROKEE Y, PASSED NEAR AND TO MY R TRAVELING IN THE OPPOSITE DIRECTION. Y WAS OBSERVED TO TAKE EVASIVE ACTION, AND THAT IS WHEN WE SPOTTED IT. I BELIEVE THE CAUSE OF THIS EVENT WAS TWO FOLD. FIRST, PLT ERROR ON THE PART OF THE PLT OF Y UPON ENTERING THE SAN CARLOS TFC PATTERN AND LACK OF GOOD JUDGEMENT. SECOND, ATC FAILING TO NOTICE THAT THIS ACFT WAS ENTERING THE TFC AREA AT THE DEP END OF THE ACTIVE RWY! NOTE: I RECOGNIZED THE ACFT IN QUESTION AND I COULD ALSO READ THE N-NUMBER AS IT PASSED OFF MY WING.
