Narrative:
Distraction in cockpit just prior to loading new set of waypoints. Aircraft came to final point and started turn back to waypoint #1. After 40 degrees of turn all 3 crew members figured out reason for the turn. A correction was initiated immediately. A backside of clock flight and a missing augmented crew member added to the fatigue level in the cockpit. Crew segment 12 hours 52 mins. Supplemental information from acn 310848: prior to last loaded INS waypoint, a flight attendant entered the cockpit (distraction). We noticed after about 45 degrees of turn at plus or minus 5 degrees of bank. This old INS doesn't consider this an error so there is no warning prior to or during this scenario.

Title: DEV FROM CLRNC RTE. ACR FLC DOES NOT ENTER THE COMPLETE RTE INTO THE INS SO IT BEGINS A SLOW TURN BACK TO THE DEP POINT AFTER IT CROSSES THE LAST ENTERED NAV POINT.
Narrative: DISTR IN COCKPIT JUST PRIOR TO LOADING NEW SET OF WAYPOINTS. ACFT CAME TO FINAL POINT AND STARTED TURN BACK TO WAYPOINT #1. AFTER 40 DEGS OF TURN ALL 3 CREW MEMBERS FIGURED OUT REASON FOR THE TURN. A CORRECTION WAS INITIATED IMMEDIATELY. A BACKSIDE OF CLOCK FLT AND A MISSING AUGMENTED CREW MEMBER ADDED TO THE FATIGUE LEVEL IN THE COCKPIT. CREW SEGMENT 12 HRS 52 MINS. SUPPLEMENTAL INFO FROM ACN 310848: PRIOR TO LAST LOADED INS WAYPOINT, A FLT ATTENDANT ENTERED THE COCKPIT (DISTR). WE NOTICED AFTER ABOUT 45 DEGS OF TURN AT PLUS OR MINUS 5 DEGS OF BANK.
