Narrative:
I was flying the airplane departing bhx, the captain was talking to ATC, the io was getting the oceanic clearance. We were very busy with vectoring, altitude restrs and minor aircraft malfunctions. Neither the captain or I monitored the clearance. The io told us the clearance was the same with one change. We entered that change in the flight plan and checked the route per procedures. The io failed to tell us that another waypoint had also changed. Shortly after crossing 5810N, shanwick asked us to confirm our 20 west position. We told them that it was 6120N. They told us that we should be going to 6020N. We corrected to course, but we had gone about 15 mi north of the proper track. The io had written the clearance down correctly, but he failed to tell us of the second change and neither the captain or I read what he had written down. We had used a verbal confirmation of the clearance with the io when we should have backed that up by reading what he had written down.

Title: FLC DEVIATES FROM ASSIGNED TRACK.
Narrative: I WAS FLYING THE AIRPLANE DEPARTING BHX, THE CAPT WAS TALKING TO ATC, THE IO WAS GETTING THE OCEANIC CLRNC. WE WERE VERY BUSY WITH VECTORING, ALT RESTRS AND MINOR ACFT MALFUNCTIONS. THE IO FAILED TO TELL US THAT ANOTHER WAYPOINT HAD ALSO CHANGED. SHORTLY AFTER XING 5810N, SHANWICK ASKED US TO CONFIRM OUR 20 W POS. WE TOLD THEM THAT IT WAS 6120N. THEY TOLD US THAT WE SHOULD BE GOING TO 6020N. WE CORRECTED TO COURSE, BUT WE HAD GONE ABOUT 15 MI N OF THE PROPER TRACK. THE IO HAD WRITTEN THE CLRNC DOWN CORRECTLY, BUT HE FAILED TO TELL US OF THE SECOND CHANGE AND NEITHER THE CAPT OR I READ WHAT HE HAD WRITTEN DOWN. WE HAD USED A VERBAL CONFIRMATION OF THE CLRNC WITH THE IO WHEN WE SHOULD HAVE BACKED THAT UP BY READING WHAT HE HAD WRITTEN DOWN.
