Narrative:
Takeoff from ctb 14900 pounds, 77 degree F, runway 23, wind calm. Normal acceleration and full power from engine. Rotated in ground effect 110 plus KTS, gear retracted -- aircraft settled to runway in spite of attempts to control this with elevator. Aircraft contacted runway level slid off at end of runway 20-30 ft in grass. Contacted runway lights, became airborne again as ground rapidly dropped away to a deep valley. Gained flying speed, gear was up, flaps in takeoff 31 degree position. Plane sank into valley, attained climb and flying speed. Climbed to 13000-14000 ft, assessed damage. Radio contact by hand held only, flaps functional, gear cycled down, nose gear trapped. Flew over airport, nose gear reported up. Exhausted fuel, landed on main gear only, held nose off, slid to stop center of runway without incident or injury. This incident could have been avoided. The plane was heavily loaded, density altitude 5100 ft. I feel I should have obtained a higher airspeed under these conditions and waited longer before retracting landing gear. The plane was capable of flying off of this airport at this temperature. I believe my technique was at fault.

Title: PLTS OF AN EX-MIL SEL JET, ALLOWED ACFT TO SETTLE BACK ONTO RWY DURING TKOF CAUSING NOSE GEAR TO BE JAMMED IN THE UP POS AND CONTINUED WITH TKOF FOR EVENTUAL NOSE GEAR UP LNDG.
