Narrative:
I do not know, nor did the company inform me, my LOA has expired. My letter was for check rides, 299 rides and IOE. My letter apparently expired jul/xx/95. The company schedulers scheduled me for 299 and IOE checks in aug/xx/95 after the apparent fact. I was not aware of any violation or breach of the FARS until I was notified oct/xx/95, by my union safety chairman. At that time I was aware of FAA involvement. Until said time I thought no violation had occurred, just a misplacement of my paperwork, LOA. Three 299 and 1 IOE had been completed in aug/xx/95. None were completed in sep/xx/95. It is the 4 incidents the company and FAA are concerned with. I did not know a letter was coming from the FAA until I talked with them oct/xd/95. I then received the letter on or about oct/xe/95 or oct/xf/95. This report is mailed within 10 days of my knowledge that a violation had possibly occurred.

Narrative: I DO NOT KNOW, NOR DID THE COMPANY INFORM ME, MY LOA HAS EXPIRED. MY LETTER WAS FOR CHK RIDES, 299 RIDES AND IOE. MY LETTER APPARENTLY EXPIRED JUL/XX/95.
