Narrative:
Cleared to taxi from the company ramp at rsw to runway 06, at approximately XX45 nov/thur/95. Supplemental information from acn 320526: the WX at rsw was excellent, with very light traffic. Our parking spot is fairly close to the takeoff end of runway 06, so we were still running the before takeoff checklist as we approached the end of the runway. Tower came on the radio as the first officer, as PF, was giving his part of the takeoff briefing, and gave us a takeoff clearance. He did not hear the tower call, since he was speaking, so when he finished I said that the tower had called us. He called them back and they said we were cleared for takeoff, plus after takeoff instructions. I heard the 'cleared for takeoff' part, but was concentrating on pulling onto the runway, checking for landing traffic, and calling for final checks, so failed to listen closely to tower's transmission. As a result, before starting the takeoff roll, I felt uncomfortable enough to request the first officer to ask the tower to reconfirm our takeoff clearance, which he did. Tower repeated the clearance for takeoff, but not any after takeoff instructions, and we started the takeoff roll. After lift- off, at 500 ft AGL, the first officer started a left turn to intercept our outbound course. I was a little surprised since most takeoff clrncs are for 'runway heading' and that was what I was expecting. Tower then called and asked us what our heading was. When we answered, they said we had been told to maintain runway heading. I believe that crew fatigue was a contributing factor. This crew had flown all night the previous night, with a tail-end re-route causing us to have minimal crew rest (less than 9 hours) in the hotel in mia. I myself got about 5 1/2 hours sleep when I usually get 8 hours. The first officer only had about 3 1/2 hours because the hotel room was too hot and too bright to sleep well.

Title: ACR FLC FAILED TO HOLD RWY HDG AFTER TKOF. FO THOUGHT THAT HE WAS CLRED ENRTE. BOTH CREW MEMBERS PLEDGED TO LISTEN TO CLRNCS MORE CAREFULLY IN THE FUTURE.

WHEN WE ANSWERED, THEY SAID WE HAD BEEN TOLD TO MAINTAIN RWY HDG. I BELIEVE THAT CREW FATIGUE WAS A CONTRIBUTING FACTOR. THIS CREW HAD FLOWN ALL NIGHT THE PREVIOUS NIGHT, WITH A TAIL-END RE-RTE CAUSING US TO HAVE MINIMAL CREW REST (LESS THAN 9 HRS) IN THE HOTEL IN MIA. I MYSELF GOT ABOUT 5 1/2 HRS SLEEP WHEN I USUALLY GET 8 HRS. THE FO ONLY HAD ABOUT 3 1/2 HRS BECAUSE THE HOTEL ROOM WAS TOO HOT AND TOO BRIGHT TO SLEEP WELL.
