Narrative:
I cleared air carrier X for takeoff on runway 32L, assigned heading 020 degrees. Aircraft read back heading 060 degrees and flew it. Separation was lost with air carrier Y who departed runway 4L on runway heading. I was giving a position relief briefing to a new controller and was receiving information on a confign change from supervisor at same time. This is why I missed readback. Ord tower TRACON has serious problems with a specific airline, air carrier X, with readback and other errors -- MD80's especially. About 80- 90 percent of all errors/deviations involve a specific airline aircraft. This 'deal' has brought major attention to the problem. I can't remember how far you guys go with this, but anything you can do to help this situation would be great. Before an accident happens! This is my first error in 10 yrs of ATC. I've caught many readback errors in the past. We have no problems with another air carrier -- why the specific air carrier? It needs to be looked into.
Original NASA ASRS Text
Title: OPERROR RPTED INVOLVING ACR X FLYING THE WRONG HDG AFTER TKOF AND CONFLICTING WITH ACR Y DEPARTING ANOTHER RWY. ACR X HAD READ BACK THE WRONG ASSIGNED HDG. THE DISTRACTED CTLR MISSED THE READBACK.
Narrative: I CLRED ACR X FOR TKOF ON RWY 32L, ASSIGNED HDG 020 DEGS. ACFT READ BACK HDG 060 DEGS AND FLEW IT. SEPARATION WAS LOST WITH ACR Y WHO DEPARTED RWY 4L ON RWY HDG. I WAS GIVING A POS RELIEF BRIEFING TO A NEW CTLR AND WAS RECEIVING INFO ON A CONFIGN CHANGE FROM SUPVR AT SAME TIME. THIS IS WHY I MISSED READBACK. ORD TWR TRACON HAS SERIOUS PROBS WITH A SPECIFIC AIRLINE, ACR X, WITH READBACK AND OTHER ERRORS -- MD80'S ESPECIALLY. ABOUT 80- 90 PERCENT OF ALL ERRORS/DEVS INVOLVE A SPECIFIC AIRLINE ACFT. THIS 'DEAL' HAS BROUGHT MAJOR ATTN TO THE PROB. I CAN'T REMEMBER HOW FAR YOU GUYS GO WITH THIS, BUT ANYTHING YOU CAN DO TO HELP THIS SIT WOULD BE GREAT. BEFORE AN ACCIDENT HAPPENS! THIS IS MY FIRST ERROR IN 10 YRS OF ATC. I'VE CAUGHT MANY READBACK ERRORS IN THE PAST. WE HAVE NO PROBS WITH ANOTHER ACR -- WHY THE SPECIFIC ACR? IT NEEDS TO BE LOOKED INTO.
