Narrative:
I was working a low altitude sector. I climbed a pit departure according to prescribed procedures and inadvertently forgot to switch the B757 frequency. 5 mins later this B757 got together with another aircraft (an MD80) in the high altitude sector.

Title: LOW ALT RADAR CTLR FORGOT TO CHANGE AN ACR B757 OVER TO HIGH ALT CTLR'S FREQ AFTER HDOF. THE B757 ENDED UP WITH LTSS WITH AN MD80 AT THE SAME ALT APPROX 5 MINS LATER.
Narrative: I WAS WORKING A LOW ALT SECTOR. I CLBED A PIT DEP ACCORDING TO PRESCRIBED PROCS AND INADVERTENTLY FORGOT TO SWITCH THE B757 FREQ. 5 MINS LATER THIS B757 GOT TOGETHER WITH ANOTHER ACFT (AN MD80) IN THE HIGH ALT SECTOR.
