Low altitude radar controller forgot to change an air carrier B757 over to high altitude controller's frequency after handoff. The B757 ended up with less than standard separation with an MD80 at the same altitude approximately 5 mins later.

I was working a low altitude sector. I climbed a pit departure according to prescribed procedures and inadvertently forgot to switch the B757 frequency. 5 mins later this B757 got together with another aircraft (an MD80) in the high altitude sector.
