Narrative:

Report concerning inadvertent operation of aircraft, a cessna 182RG, 5.3 hours beyond recommend time between overhauls (tbo). On aug/xx/96, abcd of YYY aviation, notified ZZZ aviation that an air taxi aircraft, 000, a cessna 182RG, had inadvertently been operated beyond recommended tbo, an item specified in our company operations manual. It seems that enough mathematical error in previous logbook entries, and through possible procedural gaps, that our company, ZZZ aviation operated aircraft 5.3 hours beyond recommended tbo. Abcd is our director of maintenance, owner of YYY aviation, and the owner of aircraft. All of our maintenance is through contract with YYY aviation. Aircraft is on lease to ZZZ aviation, and to this point has been operated both as a rental aircraft and as a part 135 aircraft. There were 5 legs flown involving 2 pilots, DDD, 2.7 hours, and WWW, 2.6 hours. The engine, a lycoming 0-540 has a recommended tbo of 2000 hours. Actual overhaul was begun at 2005.3 hours. Possible causes and solutions: 1) aircraft logbooks were being kept in the contract maintenance office. Since this occurrence, we have moved all logbooks into the ZZZ aviation office. 2) mathematical errors or oversights: we have hired full time, an ia to review and oversee all part 135 logbooks, maintenance compliance issues, and procedures. 3) previous aircraft flight/maintenance discrepancies log required some interpretation to determine engine overhaul times. A change has been implemented on the flight/maintenance discrepancies form to include engine overhaul due time which reflects the actual hours remaining on a daily basis. 4) engine overhaul due times were previously not included on the aircraft status board. A change has been made to include engine overhaul due on each part 135 aircraft on the status board. 5) part 135 aircraft have been utilized for rental activity. A change has been made to segregate air taxi aircraft from rental aircraft. In addition to the above policy and procedural changes, we will be monitoring all aircraft logbooks and analyzing entries more carefully.

ABCD IS OUR DIRECTOR OF MAINT, OWNER OF YYY AVIATION, AND THE OWNER OF ACFT. ALL OF OUR MAINT IS THROUGH CONTRACT WITH YYY AVIATION. ACFT IS ON LEASE TO ZZZ AVIATION, AND TO THIS POINT HAS BEEN OPERATED BOTH AS A RENTAL ACFT AND AS A PART 135 ACFT. THERE WERE 5 LEGS FLOWN INVOLVING 2 PLTS, DDD, 2.7 HRS, AND WWW, 2.6 HRS. A CHANGE HAS BEEN IMPLEMENTED ON THE FLT/MAINT DISCREPANCIES FORM TO INCLUDE ENG OVERHAUL DUE TIME WHICH REFLECTS THE ACTUAL HRS REMAINING ON A DAILY BASIS. 4) ENG OVERHAUL DUE TIMES WERE PREVIOUSLY NOT INCLUDED ON THE ACFT STATUS BOARD. A CHANGE HAS BEEN MADE TO INCLUDE ENG OVERHAUL DUE ON EACH PART 135 ACFT ON THE STATUS BOARD. 5) PART 135 ACFT HAVE BEEN UTILIZED FOR RENTAL ACTIVITY. A CHANGE HAS BEEN MADE TO SEGREGATE AIR TAXI ACFT FROM RENTAL ACFT. IN ADDITION TO THE ABOVE POLICY AND PROCEDURAL CHANGES, WE WILL BE MONITORING ALL ACFT LOGBOOKS AND ANALYZING ENTRIES MORE CAREFULLY.
