Narrative:

Forgot to turn off APU prior to takeoff. This occurred as I was non flying check airman. This occurred due to high workload. Swap of seats from left to right different flows cause the mental lapse. This is unacceptable, must find way to make sure APU or other items are connected. The flows allow one to get in good habit patterns, but make it difficult to change when changing seat.

Title: ACR MADE WITH APU OPERATING. CHECK PLT WAS OCCUPYING THE FO SEAT AND FAILED TO COMPLETE THE CHKLIST BY TURNING OFF THE APU PRIOR TO TKOF.

Narrative: FORGOT TO TURN OFF APU PRIOR TO TKOF. THIS OCCURRED AS I WAS NON FLYING CHK AIRMAN. THIS OCCURRED DUE TO HIGH WORKLOAD. SWAP OF SEATS FROM L TO R DIFFERENT FLOWS CAUSE THE MENTAL LAPSE. THIS IS UNACCEPTABLE, MUST FIND WAY TO MAKE SURE APU OR OTHER ITEMS ARE CONNECTED. THE FLOWS ALLOW ONE TO GET IN GOOD HABIT PATTERNS, BUT MAKE IT DIFFICULT TO CHANGE WHEN CHANGING SEAT.
