Narrative:
We departed molokai airport destination kona at 50 NM. Cavu on IFR flight plan. (Mkk V-7 lny vecki 5) contacted center (hnl) and cleared left turn to join V-7, maintain 7000 ft until 5 DME, then maintain 9000 ft. Clearance given and read back by PIC (PNF) approximately 4 mi south of mkk VORTAC passing through approximately 4000 ft. After passing 5 DME mkk and on course V-7 we continued climb to 9000 ft. Passing 8500 ft MSL center (hnl) called us and stated that the clearance was to maintain 7000 ft until 5 mi northwest of lny VORTAC, then to maintain 9000 ft. We were then cleared to maintain 9000 ft. Complied with the clearance issued and readback, using mkk VORTAC 5 mi as fix. Never heard, understood, or acknowledged any clearance using the lny VORTAC 5 DME fix. Greater attention by center to pilot readbacks might have prevented this misunderstanding.

Title: AN ACR MDT FLC CLBS ABOVE THEIR ASSIGNED ALT.
Narrative: WE DEPARTED MOLOKAI ARPT DEST KONA AT 50 NM. CAVU ON IFR FLT PLAN. (MKK V-7 LNY VECKI 5) CONTACTED CTR (HNL) AND CLRED L TURN TO JOIN V-7, MAINTAIN 7000 FT UNTIL 5 DME, THEN MAINTAIN 9000 FT. CLRNC GIVEN AND READ BACK BY PIC (PNF) APPROX 4 MI S OF MKK VORTAC PASSING THROUGH APPROX 4000 FT. AFTER PASSING 5 DME MKK AND ON COURSE V-7 WE CONTINUED CLB TO 9000 FT. PASSING 8500 FT MSL CTR (HNL) CALLED US AND STATED THAT THE CLRNC WAS TO MAINTAIN 7000 FT UNTIL 5 MI NW OF LNY VORTAC, THEN TO MAINTAIN 9000 FT. WE WERE THEN CLRED TO MAINTAIN 9000 FT. COMPLIED WITH THE CLRNC ISSUED AND READBACK, USING MKK VORTAC 5 MI AS FIX. NEVER HEARD, UNDERSTOOD, OR ACKNOWLEDGED ANY CLRNC USING THE LNY VORTAC 5 DME FIX. GREATER ATTN BY CTR TO PLT READBACKS MIGHT HAVE PREVENTED THIS MISUNDERSTANDING.
