Cleared into position and hold, we acknowledged, took off without clearance. No other traffic operating at the airport at the time. Early launch so fatigue a factor. Little or no communication traffic on radios, no traffic in area. Short taxi with checklists and previous maintenance problems during preflight and their effect on flight being considered during taxi.
