Narrative:
On dec xx 1996 I reported an FBO's jacksonville flight school to the FAA 800 number in washington, dc, and spoke to XXX and followed up with further information by facsimile to her office. The reason for the call was to inform the office of numerous maintenance violations for the past 8 months to a yr, which puts into question their general operating practices and oversight which may place their students at risk as well as the general public by flying substandard aircraft. Work on the jacksonville planes has been on a 'get around to it basis' with verbal notification of 'squawks' noted by the CFI's and office staff with no written documentation but only calls to the headquarters in daytona beach of an advisory nature and a fly or no-fly determination given by daytona to the jacksonville school. In the recent past no planes were available to fly in jax. Further, a CFI student from daytona scheduled a check ride with the jax FSDO chief and the plane brought from daytona was the best of what was available and was determined unairworthy by FSDO jax. The orlando FSDO to the jax FSDO maintenance officer to not set foot on FBO's ramp as they were orlando FSDO's responsibility. The 141 certificate for jax FBO has been pulled months ago, and they are operating to this day as a flight school. I am unable to determine under what far they are under to date. I was told that since I squawked too many aircraft that I was not welcome at FBO anymore. This begs the question how are squawks being reported and are they being fixed in a timely manner and under what supervision.

Title: RPT OF FBO WHO DOES NOT COMPLY WITH PROPER MAINT PROCS. RPTR HAS USED THE FAA HOTLINE TO RPT THE PROB. FBO PART 141 CERTIFICATE HAS BEEN PULLED.
Narrative: ON DEC XX 1996 I RPTED AN FBO'S JACKSONVILLE FLT SCHOOL TO THE FAA 800 NUMBER IN WASHINGTON, DC, AND SPOKE TO XXX AND FOLLOWED UP WITH FURTHER INFO BY FAX TO HER OFFICE. THE REASON FOR THE CALL WAS TO INFORM THE OFFICE OF NUMEROUS MAINT VIOLATIONS FOR THE PAST 8 MONTHS TO A YR, WHICH PUTS INTO QUESTION THEIR GENERAL OPERATING PRACTICES AND OVERSIGHT WHICH MAY PLACE THEIR STUDENTS AT RISK AS WELL AS THE GENERAL PUBLIC BY FLYING SUBSTANDARD ACFT. FURTHER, A CFI STUDENT FROM DAYTONA SCHEDULED A CHK RIDE WITH THE JAX FSDO CHIEF AND THE PLANE BROUGHT FROM DAYTONA WAS THE BEST OF WHAT WAS AVAILABLE AND WAS DETERMINED UNAIRWORTHY BY FSDO JAX. THE ORLANDO FSDO TO THE JAX FSDO MAINT OFFICER TO NOT SET FOOT ON FBO'S RAMP AS THEY WERE ORLANDO FSDO'S RESPONSIBILITY. THE 141 CERTIFICATE FOR JAX FBO HAS BEEN PULLED MONTHS AGO, AND THEY ARE OPERATING TO THIS DAY AS A FLT SCHOOL. I AM UNABLE TO DETERMINE UNDER WHAT FAR THEY ARE UNDER TO DATE. I WAS TOLD THAT SINCE I SQUAWKED TOO MANY ACFT THAT I WAS NOT WELCOME AT FBO ANYMORE. THIS BEGS THE QUESTION HOW ARE SQUAWKS BEING RPTED AND ARE THEY BEING FIXED IN A TIMELY MANNER AND UNDER WHAT SUPERVISION.
