Narrative:
Left r-hand engine running and departed aircraft. Checklists were accomplished, but reference to 'start levers-cutoff fuel flow zero' not actually observed by crew. I was sure I saw zero fuel flow but was tired and then did not actually observe start levers. After XA30 on duty all night I didn't see what I thought I saw. Fatigue reduced situational awareness -- too much in this case. Supplemental information from acn 356228: the flight to sea and back to ord was normal. We parked the aircraft, read the checklist, and went home to bed. I was soon waken up by someone wondering why we had left #2 engine running.

Title: AFTER PARKING AT THE END OF THE DUTY ASSIGNMENT. THE FLC OF A B757 RAN THE SHUTDOWN CHKLISTS AND WENT HOME TO BED LEAVING THE R ENG STILL RUNNING. DUTY SCHEDULE FATIGUE WAS CLAIMED FOR THE ERROR.
Narrative: LEFT R-HAND ENG RUNNING AND DEPARTED ACFT. CHKLISTS WERE ACCOMPLISHED, BUT REF TO 'START LEVERS-CUTOFF FUEL FLOW ZERO' NOT ACTUALLY OBSERVED BY CREW.
