Narrative:

Arriving den, first officer flying, on approach control, heard clearance to 10000 ft from 13000 ft, slow to 210 KTS and read back same for visual downwind to runway 16. Reaching 10000 ft received TCASII TA for departing B737 underneath on easterly heading and in visual contact, became RA as B737 passed on right side and below. First officer and 2 acmp new hire pilots on familiarity flight also thought 10000 ft was cleared altitude. Approach controller said we were cleared to 11000 ft. Checked ATC tapes next day, cleared to 11000 ft, read back 10000 ft, not caught by controller. No loss of separation. Departing B737 held at 9000 ft by den tower. Clearance came as aircraft approached cleared turn to 330 degree heading at 9 NM DME (hokey intersection). First officer did not turn. Captain commanded turn as clearance transmitted by approach control. Event preceded by considerable tension in cockpit over previous several days arising from belligerent and insubordinate attitude by first officer. Captain had almost removed first officer from trip that morning, but after talk with first officer decided not to remove him. First officer repeatedly challenged captain decisions, and made repeated operrors. On second day after this event, first officer made unsafe crosswind landing at mci. Problems reported to company management.

APCH CTLR ADVISED FLC THAT THEIR CLRED ALT WAS 11000 FT AND HE HAD TO STOP THE DEPARTING ACFT'S CLB AS A RESULT. RPTR CAPT ADDS THAT THE EVENT WAS PRECEDED BY SEVERAL DAYS OF DIFFICULTY WITH FO BELLIGERENT ATTITUDE AND RESULTING TENSION IN THE COCKPIT AS WELL AS THE FO PROBS WITH FLYING.
