Narrative:
On approximately jun/xa/97 I was involved with a #4 tire installation. After aligning the discs on the brake, I had packed the outer bearing and installed the tire onto the brake and torqued it per the job card. I then safetied the nut with its retaining ring and installed the hub cap. On approximately jun/xb/97 my foreman was informed that a mechanic in xyz had found the brake not engaged in the anti-rotation lugs on the wheel. The tire and brake were changed for precautionary measures and the plane returned to service. I was informed of this upon my return on jun/xc/97. To the best of my knowledge the anti-rotation lugs were engaged at the time I had signed for the work completed. I followed the job card step by step, and signed for work completed.

Title: A B737 WAS DISPATCHED WITH THE #4 WHEEL AND TIRE ASSEMBLY INCORRECTLY INSTALLED ON THE BRAKE UNIT.
Narrative: ON APPROX JUN/XA/97 I WAS INVOLVED WITH A #4 TIRE INSTALLATION. AFTER ALIGNING THE DISCS ON THE BRAKE, I HAD PACKED THE OUTER BEARING AND INSTALLED THE TIRE ONTO THE BRAKE AND TORQUED IT PER THE JOB CARD. I THEN SAFETIED THE NUT WITH ITS RETAINING RING AND INSTALLED THE HUB CAP. ON APPROX JUN/XB/97 MY FOREMAN WAS INFORMED THAT A MECH IN XYZ HAD FOUND THE BRAKE NOT ENGAGED IN THE ANTI-ROTATION LUGS ON THE WHEEL. THE TIRE AND BRAKE WERE CHANGED FOR PRECAUTIONARY MEASURES AND THE PLANE RETURNED TO SVC. I WAS INFORMED OF THIS UPON MY RETURN ON JUN/XC/97. TO THE BEST OF MY KNOWLEDGE THE ANTI-ROTATION LUGS WERE ENGAGED AT THE TIME I HAD SIGNED FOR THE WORK COMPLETED. I FOLLOWED THE JOB CARD STEP BY STEP, AND SIGNED FOR WORK COMPLETED.
