Narrative:
Aircraft #1, an airbus, departed runway 5R ind, was assigned 6000 ft MSL and turned to 270 degree heading. At approximately 4 NM wnw ind, aircraft #2, MO20 on a 350 degree heading, level at 6000 ft MSL, in handoff status from final approach position, infringed on the protected airspace of departure controller. The aircraft lost the required 3 mi separation. Aircraft #1 was turned to a 320 degree heading, aircraft #2 was turned to a 050 degree heading, then got the airbus in sight. The route of aircraft #2 took it close to the airport, on the downwind side, over aircraft on final approach. There was considerable data block overlap, and this handoff was not accomplished prior to airspace infringement on the part of aircraft #2. Aircraft #1 subsequently infringed on the protected airspace of the final controller.
Original NASA ASRS Text
Title: OPDEV RESULTING IN AN OPERROR LTSS WHEN AN MO20 IN HDOF STATUS ENTERS DEP CTLR'S AIRSPACE WITHOUT COORD AND CONFLICTS WITH AN ACR A320 AT THE SAME ALT.
Narrative: ACFT #1, AN AIRBUS, DEPARTED RWY 5R IND, WAS ASSIGNED 6000 FT MSL AND TURNED TO 270 DEG HDG. AT APPROX 4 NM WNW IND, ACFT #2, MO20 ON A 350 DEG HDG, LEVEL AT 6000 FT MSL, IN HDOF STATUS FROM FINAL APCH POS, INFRINGED ON THE PROTECTED AIRSPACE OF DEP CTLR. THE ACFT LOST THE REQUIRED 3 MI SEPARATION. ACFT #1 WAS TURNED TO A 320 DEG HDG, ACFT #2 WAS TURNED TO A 050 DEG HDG, THEN GOT THE AIRBUS IN SIGHT. THE RTE OF ACFT #2 TOOK IT CLOSE TO THE ARPT, ON THE DOWNWIND SIDE, OVER ACFT ON FINAL APCH. THERE WAS CONSIDERABLE DATA BLOCK OVERLAP, AND THIS HDOF WAS NOT ACCOMPLISHED PRIOR TO AIRSPACE INFRINGEMENT ON THE PART OF ACFT #2. ACFT #1 SUBSEQUENTLY INFRINGED ON THE PROTECTED AIRSPACE OF THE FINAL CTLR.
