Narrative:
Due to human error an electrical error occurred causing loss of radios for 3-4 mins and loss of radar for 11-12 mins. System errors may have happened. How to prevent or minimize -- do maintenance at slower traffic periods. Better backups or newer equipment. Supplemental information from acn 388863: the reason for these multiple deviations was the complete failure of all modes of radar and communication and the failure to institute approved procedures in the event of just such an emergency. This specifically would be the relinquishment of airspace to adjoining facilities per LOA's.

Title: RPT ALLEGES LOSS OF APPROPRIATE SEPARATION INVOLVING ACFT, FACILITIES AND SECTORS DUE TO ELECTRICAL FAILURE RESULTING IN LOSS OF FREQ, RADAR AND LAND LINE COM. FAILURE IS ALLEGED TO HAVE BEEN HUMAN ERROR.
Narrative: DUE TO HUMAN ERROR AN ELECTRICAL ERROR OCCURRED CAUSING LOSS OF RADIOS FOR 3-4 MINS AND LOSS OF RADAR FOR 11-12 MINS. SYS ERRORS MAY HAVE HAPPENED. HOW TO PREVENT OR MINIMIZE -- DO MAINT AT SLOWER TFC PERIODS. BETTER BACKUPS OR NEWER EQUIP.
