Narrative:
Printed flight plan stated cathedral 4 departure which called for a right turn at psp VOR. Departure was revised by clearance delivery. First officer (PF) did not change the SID displayed, flew original SID. High workload and last min changes were contributing factors. Fixation with preflight activities and terrain concerns were also factors.

Title: RPTR SO RPTS THAT THE SID HAD BEEN REVISED, BUT THE OLD SID WAS INADVERTENTLY STILL USED. RPTR ATTRIBUTES MISTAKE TO HIGH WORKLOAD, LAST MIN CHANGES, PREFLT ACTIVITIES AND TERRAIN CONCERNS.
Narrative: PRINTED FLT PLAN STATED CATHEDRAL 4 DEP WHICH CALLED FOR A R TURN AT PSP VOR. DEP WAS REVISED BY CLRNC DELIVERY. FO (PF) DID NOT CHANGE THE SID DISPLAYED, FLEW ORIGINAL SID.
