Narrative:

On mar/xx/98 and mar/xy/98 I was the lead amt assigned to aircraft xyz and aircraft XXX. Aircraft XXX was a B-1 check which required my attention in assigning manpwr, insuring work completion and monitoring paperwork. I had assigned 2 mechanics to work the variable stator vane rig on aircraft xyz. On mar/xy/98, after a power assurance run on aircraft xyz, we were clearing up the paperwork on xyz when a mechanic noticed that there were a couple of missing write-ups. There were no open write-ups in the computer and it was assumed that we were working with a paperless airplane. I redid the envelope to include the task oriented document job card and disposed of the old envelope. Later, I found out that the paperwork was being monitored by the FAA. We received a turnover on the job which involved only a monologue of what had been changed the previous shift, a boroscope inspection, and that maintenance control wanted a rerig of the variable stator vanes based on that turnover and the training I had received on a paperless airplane. Given past practices at air carrier, I felt what I did was acceptable practice regarding a paperless airplane.

Title: A B757-200 HAD WORK ACCOMPLISHED THAT WAS NOT DOCUMENTED UNTIL THE JOB WAS COMPLETED.

Narrative: ON MAR/XX/98 AND MAR/XY/98 I WAS THE LEAD AMT ASSIGNED TO ACFT XYZ AND ACFT XXX. ON MAR/XY/98, AFTER A PWR ASSURANCE RUN ON ACFT XYZ, WE WERE CLRING UP THE PAPERWORK ON XYZ WHEN A MECH NOTICED THAT THERE WERE A COUPLE OF MISSING WRITE-UPS. THERE WERE NO OPEN WRITE-UPS IN THE COMPUTER AND IT WAS ASSUMED THAT WE WERE WORKING WITH A PAPERLESS AIRPLANE. I REDID THE ENVELOPE TO INCLUDE THE TASK ORIENTED DOCUMENT JOB CARD AND DISPOSED OF THE OLD ENVELOPE. LATER, I FOUND OUT THAT THE PAPERWORK WAS BEING MONITORED BY THE FAA. GIVEN PAST PRACTICES AT ACR, I FELT WHAT I DID WAS ACCEPTABLE PRACTICE REGARDING A PAPERLESS AIRPLANE.
