Narrative:
On xa april 1998 I was the lead technician on my shift. Aircraft xyz, flight YYY, arrived with a reverser problem. 'Left reverser did not deploy until aircraft reached 80 KTS.' the aircraft had previous reverser problems and I decided that the time required to troubleshoot and fix the problem would cause the flight a substantial delay. Per air carrier regulation, MEL, one reverser is allowed to be deactivated. I assigned three technicians to physically lock out the left reverser and I made the deferred item in the computer. The aircraft flew five legs. After its fifth landing at yyz, it was reported that the flight crew deployed the thrust reversers. Half of the left reverser deployed causing damage to the reverser half. The aircraft was removed from service and the reverser half replaced. Aircraft xyz is now back in regular passenger service.

Title: A B767-300 WAS DISPATCHED WITH THE #1 ENGINE REVERSER DEFERRED INOPERATIVE BUT WAS NOT LOCKED CORRECTLY IN THE FORWARD THRUST POSITION PER THE MEL SPECIAL PROCEDURES.
Narrative: ON XA APRIL 1998 I WAS THE LEAD TECHNICIAN ON MY SHIFT. ACFT XYZ, FLT YYY, ARRIVED WITH A REVERSER PROBLEM. 'L REVERSER DID NOT DEPLOY UNTIL ACFT REACHED 80 KTS.' THE ACFT HAD PREVIOUS REVERSER PROBLEMS AND I DECIDED THAT THE TIME REQUIRED TO TROUBLESHOOT AND FIX THE PROBLEM WOULD CAUSE THE FLT A SUBSTANTIAL DELAY. PER ACR REGULATION, MEL, ONE REVERSER IS ALLOWED TO BE DEACTIVATED. I ASSIGNED THREE TECHNICIANS TO PHYSICALLY LOCK OUT THE L REVERSER AND I MADE THE DEFERRED ITEM IN THE COMPUTER. THE ACFT FLEW FIVE LEGS. AFTER ITS FIFTH LNDG AT YYZ, IT WAS REPORTED THAT THE FLT CREW DEPLOYED THE THRUST REVERSERS. HALF OF THE L REVERSER DEPLOYED CAUSING DAMAGE TO THE REVERSER HALF. THE ACFT WAS REMOVED FROM SERVICE AND THE REVERSER HALF REPLACED. ACFT XYZ IS NOW BACK IN REGULAR PAX SERVICE.
