Narrative:
4 skydivers exited the aircraft separately at 3-7 second intervals. Exit altitude was 10000 ft AGL. The 4TH and last to exit the aircraft failed to deploy her main or reserve parachute and impacted the ground. The skydiver did not survive the impact, and on initial investigation it was discovered that the reserve was out of the 120 day repack window by 6 days.

Title: SKYDIVER FAILS TO OPEN PARACHUTE AFTER JUMP FROM C182 IN PDX AIRSPACE.
Narrative: 4 SKYDIVERS EXITED THE ACFT SEPARATELY AT 3-7 SECOND INTERVALS. EXIT ALT WAS 10000 FT AGL. THE 4TH AND LAST TO EXIT THE ACFT FAILED TO DEPLOY HER MAIN OR RESERVE PARACHUTE AND IMPACTED THE GND. THE SKYDIVER DID NOT SURVIVE THE IMPACT, AND ON INITIAL INVESTIGATION IT WAS DISCOVERED THAT THE RESERVE WAS OUT OF THE 120 DAY REPACK WINDOW BY 6 DAYS.
