Narrative:

Received clearance from approach control to descend from 7000 ft to 3000 ft and turn to heading 040 degrees. I read back that clearance and entered 3000 ft into the altitude reminder. The first officer confirmed the altitude. After flying at 3000 ft for about 5 mins, approach control told us to change to frequency 134.9, and asked us if we were at 3000 ft. I confirmed 3000 ft. He told us we should be at 4000 ft. He then told us to maintain 3000 ft and change to 134.9.

Narrative: RECEIVED CLRNC FROM APCH CTL TO DSND FROM 7000 FT TO 3000 FT AND TURN TO HDG 040 DEGS. I READ BACK THAT CLRNC AND ENTERED 3000 FT INTO THE ALT REMINDER. THE FO CONFIRMED THE ALT. AFTER FLYING AT 3000 FT FOR ABOUT 5 MINS, APCH CTL TOLD US TO CHANGE TO FREQ 134.9, AND ASKED US IF WE WERE AT 3000 FT. I CONFIRMED 3000 FT. HE TOLD US WE SHOULD BE AT 4000 FT. HE THEN TOLD US TO MAINTAIN 3000 FT AND CHANGE TO 134.9.
