Narrative:
Our company operations manual and MEL books have no published procedures for flcs to follow to properly accomplish deferring a minor maintenance item at an off-line station with limited or no maintenance service. We had an item, which is not required by the FARS (first aid kit ice pack) which was used on the ground by the flight attendants for a passenger. I entered this in the log and contacted our maintenance controller via a remote communications facility. It was a bad phone link -- hard to understand. In the end, I filled out the corrective actions portion of the log per their instructions. However, I neglected to get a name and zulu date and time for this deferral as it was not offered, nor did I realize, required. I have submitted a report to the company to add this information and suggest they add to our flight operations manual a section on deferrals in addition to our en route MEL procedures for flcs.

Title: AN MD83 WAS DISPATCHED WITH A LOG ITEM DEFERRED IMPROPERLY. ITEM HAD NO NAME AUTHORIZING DEFERRAL AND NO DATE OF DEFERRAL.

I ENTERED THIS IN THE LOG AND CONTACTED OUR MAINT CTLR VIA A REMOTE COMS FACILITY. IT WAS A BAD PHONE LINK -- HARD TO UNDERSTAND.
