Narrative:
After installing a hydraulic line it was brought to my attention that a 3/4 inch wrench bearing my personal initials had fallen from aircraft xyz in a parking lot during takeoff. I believe that fatigue and inadequate lighting were contributing factors.

Title: A DC9-30 WAS DISPATCHED AFTER MAINT WITH A WRENCH LEFT IN THE R WING ROOT.
Narrative: AFTER INSTALLING A HYD LINE IT WAS BROUGHT TO MY ATTN THAT A 3/4 INCH WRENCH BEARING MY PERSONAL INITIALS HAD FALLEN FROM ACFT XYZ IN A PARKING LOT DURING TKOF. I BELIEVE THAT FATIGUE AND INADEQUATE LIGHTING WERE CONTRIBUTING FACTORS.
