Narrative:
Flight was a routine trip from phx to ont. Due to departure familiarity, the captain's takeoff brief was abbreviated. On departure, the squawk was incorrect and departure control asked us to change it as the FMC entered 'low altitude capture' mode. The first officer became task saturated as he attempted to set the aircraft speed bug to clean maneuver speed and change the IFF. The aircraft accelerated to approximately 290 KTS at 3500 ft MSL. Additionally, lateral navigation guidance in LNAV showed us slightly left of the buckey 2 SID 240 degree heading. At this point, approximately 8 mi from phx, the captain, seeing the command bars indicating a right turn, commenced a right turn to avona. Departure control directed us back to a 235 degree heading. Complacency caused by familiarity, distraction caused by the squawk and misinterp of command bars caused our deviations.

Title: FLC OF A B767 FAILED TO TURN TO A REQUIRED SID HDG ON DEP, AND EXCEEDED MAX AIRSPD BELOW 10000 FT.
Narrative: FLT WAS A ROUTINE TRIP FROM PHX TO ONT. DUE TO DEP FAMILIARITY, THE CAPT'S TKOF BRIEF WAS ABBREVIATED. ON DEP, THE SQUAWK WAS INCORRECT AND DEP CTL ASKED US TO CHANGE IT AS THE FMC ENTERED 'LOW ALT CAPTURE' MODE. THE FO BECAME TASK SATURATED AS HE ATTEMPTED TO SET THE ACFT SPD BUG TO CLEAN MANEUVER SPD AND CHANGE THE IFF. THE ACFT ACCELERATED TO APPROX 290 KTS AT 3500 FT MSL. ADDITIONALLY, LATERAL NAV GUIDANCE IN LNAV SHOWED US SLIGHTLY L OF THE BUCKEY 2 SID 240 DEG HDG. AT THIS POINT, APPROX 8 MI FROM PHX, THE CAPT, SEEING THE COMMAND BARS INDICATING A R TURN, COMMENCED A R TURN TO AVONA. DEP CTL DIRECTED US BACK TO A 235 DEG HDG. COMPLACENCY CAUSED BY FAMILIARITY, DISTR CAUSED BY THE SQUAWK AND MISINTERP OF COMMAND BARS CAUSED OUR DEVS.
