Narrative:
Aircraft would not pressurize. On takeoff, on before takeoff check, noted aft cargo door light. Consulted MEL, which said '...if it can be verified that aircraft will pressurize flight may be continued.' we selected manual pressurization and closed outflow valve. Cabin pressure started to descend at 500 FPM. We believed this indicated cabin would pressurize. However, there is no guidance on how to 'verify cabin will pressurize.' we obviously erred in continuing takeoff since aircraft didn't pressurize and upon return to gate the aft cargo door was open. From a human factors point of view, we question when the door light came on. Was it on at pushback and we just missed it? Possibly! One of our EPR gauges was inoperative, which required a multi-step process in the odm to calculate V speeds. This distraction may have caused us to miss the aft warning door message.

Title: AN MD88 ON INITIAL CLB AT 1000 FT RETURN LAND DUE TO UNABLE TO PRESSURIZE THE CABIN CAUSED BY THE AFT CARGO DOOR OPEN.
Narrative: ACFT WOULD NOT PRESSURIZE. ON TKOF, ON BEFORE TKOF CHK, NOTED AFT CARGO DOOR LIGHT. CONSULTED MEL, WHICH SAID '...IF IT CAN BE VERIFIED THAT ACFT WILL PRESSURIZE FLT MAY BE CONTINUED.'
