Narrative:

Both engines were removed due to metal contamination found in oil filters at inspection. Logbook entries were made stating the engines were removed. The engines were sent to the overhaul facility where they had been overhauled 300 flight hours prior. In subsequent phone calls, the overhaul facility indicated the engines had been disassembled and inspected and nothing abnormal was found. The engines were returned to us and installed on the aircraft. Historically, this overhaul facility sends their logbook entries directly to the flying club stating their disassembly, inspection, and any repairs made. The owner of this aircraft was in communication with the overhaul facility discussing this work being covered as warranty work. It was agreed that the logbook entries would be sent to the owner. Space was left in the logbooks to insert the logbook entries when received. The engine installation was entered into the logbooks and signed off. During a later preflight review of the logbooks, the pilot discovered that the log entries from the overhaul facility had not yet been made. The overhaul facility was contacted by phone, and at that time it was discovered that, in fact, only 1 of the engines had been disassembled and checked. Since only 1 of the engines was disassembled and inspected, we technically have no maintenance release on the second engine, which was reinstalled on the aircraft. The aircraft has subsequently been grounded as it continued to show metal contamination. As of this writing, we have received a faxed copy of work done by the overhaul facility and the log entries have been sent to the owner.

Title: A PA44-180T WAS DISPATCHED AND OPERATED IN NON COMPLIANCE WITH AN OPEN LOG ITEM ON ENG WORK ACCOMPLISHED BUT NOT ENTERED IN THE LOGBOOK.

DURING A LATER PREFLT REVIEW OF THE LOGBOOKS, THE PLT DISCOVERED THAT THE LOG ENTRIES FROM THE OVERHAUL FACILITY HAD NOT YET BEEN MADE. THE OVERHAUL FACILITY WAS CONTACTED BY PHONE, AND AT THAT TIME IT WAS DISCOVERED THAT, IN FACT, ONLY 1 OF THE ENGS HAD BEEN DISASSEMBLED AND CHKED. SINCE ONLY 1 OF THE ENGS WAS DISASSEMBLED AND INSPECTED, WE TECHNICALLY HAVE NO MAINT RELEASE ON THE SECOND ENG, WHICH WAS REINSTALLED ON THE ACFT. THE ACFT HAS SUBSEQUENTLY BEEN GNDED AS IT CONTINUED TO SHOW METAL CONTAMINATION. AS OF THIS WRITING, WE HAVE RECEIVED A FAXED COPY OF WORK DONE BY THE OVERHAUL FACILITY AND THE LOG ENTRIES HAVE BEEN SENT TO THE OWNER.
