Air Traffic Incident | Pilot Deviation |
Narrative:
Air carrier flight syr-clt shine 5 arrival, approaching hmv, assigned heading off of arrival (uncertain of heading). After 2-3 mins, assigned a new heading of 200 degrees, I think, and then told to intercept 314 degree radial from clt VOR and resume shine arrival. I mistakenly set up intercept on 156 degree radial from hmv on FMC. After a short time, ZTL noticed deviation, told me of my mistake and issued a new clearance. Flight was completed uneventfully. I don't know if there were any traffic conflicts with other aircraft. Solution: be more careful with automated flight guidance system. Company policy is for both pilots to agree before a course change is 'executed' in the FMC. Captain also failed to detect error.
Original NASA ASRS Text
Title: FO OF A B737-300 FAILED TO REINTERCEPT THE STAR ARR RTE AFTER ATC VECTOR DUE MISPROGRAMMING THE FMC ON THE WRONG RADIAL AND VORTAC. ATC INTERVENED AND PROVIDED A NEW CLRNC.
Narrative: ACR FLT SYR-CLT SHINE 5 ARR, APCHING HMV, ASSIGNED HDG OFF OF ARR (UNCERTAIN OF HDG). AFTER 2-3 MINS, ASSIGNED A NEW HDG OF 200 DEGS, I THINK, AND THEN TOLD TO INTERCEPT 314 DEG RADIAL FROM CLT VOR AND RESUME SHINE ARR. I MISTAKENLY SET UP INTERCEPT ON 156 DEG RADIAL FROM HMV ON FMC. AFTER A SHORT TIME, ZTL NOTICED DEV, TOLD ME OF MY MISTAKE AND ISSUED A NEW CLRNC. FLT WAS COMPLETED UNEVENTFULLY. I DON'T KNOW IF THERE WERE ANY TFC CONFLICTS WITH OTHER ACFT. SOLUTION: BE MORE CAREFUL WITH AUTOMATED FLT GUIDANCE SYS. COMPANY POLICY IS FOR BOTH PLTS TO AGREE BEFORE A COURSE CHANGE IS 'EXECUTED' IN THE FMC. CAPT ALSO FAILED TO DETECT ERROR.
