Narrative:
Chaotic preflight and WX out of boston. Pre departure clearance stated route clearance without SID. 4 lines of instruction added to bottom of pre departure clearance. Missed logan 2 departure included on bottom of pre departure clearance. Bos departure frequency queried us as to our heading and we replied runway heading. ATC instructed us to come right to heading 120 degrees. No mention of error was made. We realized our mistake later on climb out. In our haste we overlooked all the notes on the pre departure clearance. Will not happen again.
