Narrative:
On apr/xa/99 at approximately AB00, a mechanic was accomplishing a postflt walkaround on aircraft xyz. While approaching aft left wing, he noted a spoiler board slightly extended. Upon closer examination, it was determined that the jam nut on #5 spoiler actuator had come loose and permitted the actuator to loose the setting on the board. We then turned the actuator to a nominal setting and deactivated the #5 actuator and the corresponding actuator on the right wing in accordance with MEL. At this time some confusion occurred concerning the numbering of the actuators and the MEL was issued against actuators #4 and #9 rather than the ones that had been deactivated (#5 and #8). The aircraft was dispatched at this time. The follow-up performed by maintenance at the next station produced disparity in the MEL.

Title: A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH GND SPOILERS #4 AND #9 ENTERED IN LOGBOOK AS DEACTIVATED PER THE MEL. ACTUAL SPOILERS DEACTIVATED WERE GND SPOILERS #5 AND #8.
Narrative: ON APR/XA/99 AT APPROX AB00, A MECH WAS ACCOMPLISHING A POSTFLT WALKAROUND ON ACFT XYZ. WHILE APCHING AFT L WING, HE NOTED A SPOILER BOARD SLIGHTLY EXTENDED. UPON CLOSER EXAMINATION, IT WAS DETERMINED THAT THE JAM NUT ON #5 SPOILER ACTUATOR HAD COME LOOSE AND PERMITTED THE ACTUATOR TO LOOSE THE SETTING ON THE BOARD. WE THEN TURNED THE ACTUATOR TO A NOMINAL SETTING AND DEACTIVATED THE #5 ACTUATOR AND THE CORRESPONDING ACTUATOR ON THE R WING IN ACCORDANCE WITH MEL. AT THIS TIME SOME CONFUSION OCCURRED CONCERNING THE NUMBERING OF THE ACTUATORS AND THE MEL WAS ISSUED AGAINST ACTUATORS #4 AND #9 RATHER THAN THE ONES THAT HAD BEEN DEACTIVATED (#5 AND #8). THE ACFT WAS DISPATCHED AT THIS TIME. THE FOLLOW-UP PERFORMED BY MAINT AT THE NEXT STATION PRODUCED DISPARITY IN THE MEL.
