Narrative:
Upon our arrival at our destination, I became aware that we had transported as checked baggage a battery-pwred mobility aid. I did not receive a 'special load notification to captain' form as required by our company's fom. Upon further investigation it was also brought to my attention that the battery terminals had not been disconnected/insulated to prevent accidental short circuits in accordance with the fom. I contacted my flight manager to inform him of this occurrence. The flight manager forwarded the information I provided to the company internal safety committee for their review and subsequent investigation.

Title: AN ACR FLC FLYING DC9 DISCOVERS AFTER LNDG AT OMA THAT HAZMAT HAD BEEN LOADED ON THEIR ACFT WITHOUT THE PROPER CREW NOTIFICATION OR PRECAUTION.
Narrative: UPON OUR ARR AT OUR DEST, I BECAME AWARE THAT WE HAD TRANSPORTED AS CHKED BAGGAGE A BATTERY-PWRED MOBILITY AID. I DID NOT RECEIVE A 'SPECIAL LOAD NOTIFICATION TO CAPT' FORM AS REQUIRED BY OUR COMPANY'S FOM. UPON FURTHER INVESTIGATION IT WAS ALSO BROUGHT TO MY ATTN THAT THE BATTERY TERMINALS HAD NOT BEEN DISCONNECTED/INSULATED TO PREVENT ACCIDENTAL SHORT CIRCUITS IN ACCORDANCE WITH THE FOM. I CONTACTED MY FLT MGR TO INFORM HIM OF THIS OCCURRENCE. THE FLT MGR FORWARDED THE INFO I PROVIDED TO THE COMPANY INTERNAL SAFETY COMMITTEE FOR THEIR REVIEW AND SUBSEQUENT INVESTIGATION.
