Narrative:

Making an approach into lbe, PA, ZOB clears us for the ILS runway 23 with an advisory to contact the tower established on the approach. The first officer read back the clearance and tuned to what he thought was tower. (The aircraft had just been upgraded with the 8.33 UHF spacing and the frequency presentation is different than previous. This was the first officer's first time in the airplane after the radio update and he was getting used to the new presentation.) meanwhile, the localizer and GS came in rapidly and we intercept and configure and start the descent on the GS still in IFR conditions. The first officer changes the frequency and gives the tower a call. He immediately starts bleeding from the nose quite profusely (he thinks from the pressure change/WX change). I am flying the approach and he is looking for something to stop the blood flow. The frequency he has selected is incorrect and we break out on 3 mi final and land without clearance from the tower. Upon clearing the runway, we see the red light from the tower, re-tune the radio and establish contact.

Narrative: MAKING AN APCH INTO LBE, PA, ZOB CLRS US FOR THE ILS RWY 23 WITH AN ADVISORY TO CONTACT THE TWR ESTABLISHED ON THE APCH. THE FO READ BACK THE CLRNC AND TUNED TO WHAT HE THOUGHT WAS TWR. (THE ACFT HAD JUST BEEN UPGRADED WITH THE 8.33 UHF SPACING AND THE FREQ PRESENTATION IS DIFFERENT THAN PREVIOUS. THIS WAS THE FO'S FIRST TIME IN THE AIRPLANE AFTER THE RADIO UPDATE AND HE WAS GETTING USED TO THE NEW PRESENTATION.) MEANWHILE, THE LOC AND GS CAME IN RAPIDLY AND WE INTERCEPT AND CONFIGURE AND START THE DSCNT ON THE GS STILL IN IFR CONDITIONS. THE FO CHANGES THE FREQ AND GIVES THE TWR A CALL. HE IMMEDIATELY STARTS BLEEDING FROM THE NOSE QUITE PROFUSELY (HE THINKS FROM THE PRESSURE CHANGE/WX CHANGE). I AM FLYING THE APCH AND HE IS LOOKING FOR SOMETHING TO STOP THE BLOOD FLOW. THE FREQ HE HAS SELECTED IS INCORRECT AND WE BREAK OUT ON 3 MI FINAL AND LAND WITHOUT CLRNC FROM THE TWR. UPON CLRING THE RWY, WE SEE THE RED LIGHT FROM THE TWR, RE-TUNE THE RADIO AND ESTABLISH CONTACT.
