Narrative:
Both occurrences are related and can be attributed to quip malfunction or mishandling, and impaired communication between the PF and the PNF. During the first occurrence we were climbing and were given a leveloff at 4000 ft. I failed to notice that we had arrived at 4000 ft. In addition, the altitude alerter was off and failed to alert the PF. The second occurrence took place while on descent. We were issued a restr to maintain FL310 65 NM east of gipper. We crossed at FL350 instead of FL310. Both occurrences were reason for discussion throughout which we developed better CRM procedures and the continuing flts were uneventful.

Title: A GULFSTREAM II FLC FAILED TO CROSS INTXN AT ASSIGNED ALT ON DSCNT INTO ORD.
Narrative: BOTH OCCURRENCES ARE RELATED AND CAN BE ATTRIBUTED TO QUIP MALFUNCTION OR MISHANDLING, AND IMPAIRED COM BTWN THE PF AND THE PNF. BOTH OCCURRENCES WERE REASON FOR DISCUSSION THROUGHOUT WHICH WE DEVELOPED BETTER CRM PROCS AND THE CONTINUING FLTS WERE UNEVENTFUL.
