Narrative:

On jun/xa/99 I was assigned to do a final inspection of the installation of the engine driven fuel pump. I witnessed all seals installed prior to the mechanic installing the pump also. After pump was replaced and inspected, engine was run for several mins at idle and high power with no leaks detected. Aircraft was returned to service. Aircraft was dispatched on a ferry flight to XXX. 1 hour 20 mins after departing, the engine had a loss of oil and the crew did an in-flight shutdown of that engine (#2). On later removal of the replaced pump, the removing mechanics claimed that no pump driven shaft o-rings were noticed.

Title: A B727-200 IN CRUISE DECLARED AN EMER DUE TO LOSS OF #2 ENG OIL QUANTITY AND PRESSURE CAUSED BY THE ENG DRIVEN FUEL PUMP DRIVE SEALS NOT INSTALLED DURING PUMP REPLACEMENT.

Narrative: ON JUN/XA/99 I WAS ASSIGNED TO DO A FINAL INSPECTION OF THE INSTALLATION OF THE ENG DRIVEN FUEL PUMP. I WITNESSED ALL SEALS INSTALLED PRIOR TO THE MECH INSTALLING THE PUMP ALSO. AFTER PUMP WAS REPLACED AND INSPECTED, ENG WAS RUN FOR SEVERAL MINS AT IDLE AND HIGH PWR WITH NO LEAKS DETECTED. ACFT WAS RETURNED TO SVC. ACFT WAS DISPATCHED ON A FERRY FLT TO XXX. 1 HR 20 MINS AFTER DEPARTING, THE ENG HAD A LOSS OF OIL AND THE CREW DID AN INFLT SHUTDOWN OF THAT ENG (#2). ON LATER REMOVAL OF THE REPLACED PUMP, THE REMOVING MECHS CLAIMED THAT NO PUMP DRIVEN SHAFT O-RINGS WERE NOTICED.
