Narrative:

On taxi in at jfk, the R-1 door opened and slide deployed. No injuries resulted. ZZZ-jfk. Before leaving ZZZ we had trouble closing R-1. Maintenance was called and a logbook entry made. Maintenance opened and closed the door several times, then declared the door was closed and locked with the slide armed. The door indication system was put on mco and a placard placed by the door light system. Maintenance left the aircraft and we departed for nyc. On climb out the flight attendant at R-1 said the door was making more noise than usual. At leveloff, at FL310, the door made a loud bang as the door seated itself in the door opening. The company was called on satcom and told of the noises coming from R-1. All concurred that the door was closed and safe to continue. On landing at jfk the flight attendant at R-1 said he watched after landing as the door slowly opened to 18 inches then opened all the way and the slide deployed. Callback conversation with reporter revealed the following information: the reporter stated the maintenance action taken at jfk was to tighten the door cable and completely rerig the door. The reporter said the slide was not salvageable and was replaced.
