Narrative:
On jul/xa/99, I witnessed 2 tire changes and the torque, along with side play check. I failed to check to ensure that the inner spacer was in place prior to the wheel being placed on the axle. A rotation check was made while torquing to 500 plus pound initial and 150 pounds final torque. No side play was noted. One thread on the axle was noted.

Title: A B767-300 WAS DISPATCHED IN NON COMPLIANCE WITH 2 MAIN TIRE AND WHEEL ASSEMBLIES REPLACED WITHOUT THE INNER WHEEL SPACER INSTALLED.
Narrative: ON JUL/XA/99, I WITNESSED 2 TIRE CHANGES AND THE TORQUE, ALONG WITH SIDE PLAY CHK. I FAILED TO CHK TO ENSURE THAT THE INNER SPACER WAS IN PLACE PRIOR TO THE WHEEL BEING PLACED ON THE AXLE. A ROTATION CHK WAS MADE WHILE TORQUING TO 500 PLUS LB INITIAL AND 150 LBS FINAL TORQUE. NO SIDE PLAY WAS NOTED. ONE THREAD ON THE AXLE WAS NOTED.
