Narrative:
Right engine failed on takeoff at vr. Declared emergency and returned to tlv. Landed overweight for safety. All 3 crew members used to complete all relevant checklists. Supplemental information from acn 446069: shortly after rotation, heard a loud 'bang,' saw a flash of orange light out right windows and felt strong vibrations. I then noticed right egt gauge indicate red and other indications of surging engine/compressor stall. We flew engine-out procedure straight-out and captain declared an emergency with tower, and requested immediate return to airport with equipment standing by for us. We were cleared to land on any runway. After completion of engine failure, engine inoperative approach, descent/approach and landing checklists, we briefed the evacuate/evacuation checklist. Flight service manager was kept in loop the whole time and kept flight attendants (and passenger) well informed and prepared. We landed overweight, with a high approach speed. Touchdown was very smooth and we rolled to end of runway and turned off onto another runway. Equipment was waiting and informed us that right engine had large hole blown through inboard side, fan blades destroyed and our brakes were orange, but no smoke or fire. After stopping, 7 of 8 main tires went flat as fuse plugs blew. I suspect foreign object damage was responsible.

Title: A BOEING 767-300 HAD AN ENG FAILURE ON TKOF FROM TLV, FO.
Narrative: R ENG FAILED ON TKOF AT VR. DECLARED EMER AND RETURNED TO TLV. LANDED OVERWT FOR SAFETY. ALL 3 CREW MEMBERS USED TO COMPLETE ALL RELEVANT CHKLISTS. SUPPLEMENTAL INFO FROM ACN 446069: SHORTLY AFTER ROTATION, HEARD A LOUD 'BANG,' SAW A FLASH OF ORANGE LIGHT OUT R WINDOWS AND FELT STRONG VIBRATIONS. I THEN NOTICED R EGT GAUGE INDICATE RED AND OTHER INDICATIONS OF SURGING ENG/COMPRESSOR STALL.
