Narrative:
Phoenix karlo 7 STAR, cross plsnt intersection at 250 KTS and 12000 ft. The B757 normal profile is to use vertical navigation with managed speed. The first officer selected 290 KTS and then programmed 290 KTS in the FMC. The first officer forgot to reselect managed speed so the B757 would slow to 250 KTS at plsnt intersection. The B757 crossed plsnt intersection at 290 KTS (selected speed) not VNAV speed 250 KTS. Captain discovered the 40 KT airspeed error 1 mi prior to plsnt intersection. Captain fatigue (ie, day 6 of day/night mix of trips -- no sleep) and first officer (first flight in 4 weeks of no flying and new hire). Captain had flown 220 hours in last 90 days and this was the 6TH day in a row of flying reserves with day and night mixed flying. Captain slept on deadhead flight to fly the trip to phoenix. Pilot fatigued and the trips were legal according to the FARS.
