Narrative:

On oct/fri/99, after landing on runway 6 at exx davidson county airport lexington, nc, the student, mr west, gave the plane full throttle and just prior to rotation, at 55 mph IAS, the student jammed in the left rudder peddle. The plane veered off the runway at a 45 degree angle to the runway and went down a 15 degree grade embankment. As the plane slowed to approximately 5 mph the nose gear got stuck in the mud and the plane tipped forward causing a propeller strike in the mud and the right wingtip to sustain some damage. The FAA was contacted and came to the scene to investigate. As defined per the FARS, I do not believe this meets the criteria of an accident. Mr X and an a&P flew the plane back to int from exx on an FAA ferry permit. Mr X told me he believed after inspection that the plane had not sustained structural damage. I have left phone messages with the winston-salem FSDO for mr Y and mr Z, on oct/mon/99, and oct/tue/99, asking if I needed to do anything further. They have not yet responded to my phone inquiries.
