Narrative:
An MEL for the pressurization system 'rate of climb' indicator was put on the aircraft just before departure. I checked the aircraft logbook to make sure that it, the MEL, had been entered in the logbook correctly and that the preflight had been signed off, too. Then I checked for any special procedures. There were none. When we ran the checklist, one of the checks is 'papers?' both the first officer and I forgot the MEL for the pressurization system needing to be added to the flight release. This is where I should have made a note that we needed an amendment to the flight release. It is required that the flight crew and the dispatcher agree and accept amendments to the flight, in this case before the flight takes off. That did not happen.

Title: A B737-400 WAS DISPATCHED IN NON COMPLIANCE WITH A DEFERRED ITEM ENTERED IN THE LOGBOOK BUT THE MAINT RELEASE WAS NOT AMENDED TO SHOW THE DEFERRED ITEM.
Narrative: AN MEL FOR THE PRESSURIZATION SYS 'RATE OF CLB' INDICATOR WAS PUT ON THE ACFT JUST BEFORE DEP. I CHKED THE ACFT LOGBOOK TO MAKE SURE THAT IT, THE MEL, HAD BEEN ENTERED IN THE LOGBOOK CORRECTLY AND THAT THE PREFLT HAD BEEN SIGNED OFF, TOO. THEN I CHKED FOR ANY SPECIAL PROCS. THERE WERE NONE. WHEN WE RAN THE CHKLIST, ONE OF THE CHKS IS 'PAPERS?' BOTH THE FO AND I FORGOT THE MEL FOR THE PRESSURIZATION SYS NEEDING TO BE ADDED TO THE FLT RELEASE. THIS IS WHERE I SHOULD HAVE MADE A NOTE THAT WE NEEDED AN AMENDMENT TO THE FLT RELEASE. IT IS REQUIRED THAT THE FLC AND THE DISPATCHER AGREE AND ACCEPT AMENDMENTS TO THE FLT, IN THIS CASE BEFORE THE FLT TAKES OFF. THAT DID NOT HAPPEN.
