Narrative:

Upon my arrival home at XA00 on nov/sun/99, my wife handed me a registered letter postmarked nov/xx/99 and received over the wkend of nov/fri-sat/99. The letter was from an aviation safety inspector. He advised me that he had inspected on arrival the aircraft that I commanded for flight returning to dfw from kix on sep/xx/99. The inspection revealed that the cockpit door lock had failed and he was unable to secure the cockpit door. He noted no entry concerning the door had been made in the logbook. During the flight I returned to the cockpit at least 6 times and on each entry I needed to use my cockpit door key to gain entry. My last entry was approximately 45 mins prior to landing and at that time the cockpit door lock still required a key for entry. When I signed the logbook upon arrival, I believed that the cockpit door lock was operative. No flight crew member advised me during the flight or at its conclusion that they had experienced any malfunction of the cockpit door lock.

Title: CAPT OF A B777 RECEIVED LETTER OF INVESTIGATION FROM THE FAA REGARDING THE LACK OF ACFT LOGBOOK ENTRY COVERING A COCKPIT DOOR LOCK MALFUNCTION NOTED BY AN FAA INSPECTOR ALMOST 2 MONTHS EARLIER.

Narrative: UPON MY ARR HOME AT XA00 ON NOV/SUN/99, MY WIFE HANDED ME A REGISTERED LETTER POSTMARKED NOV/XX/99 AND RECEIVED OVER THE WKEND OF NOV/FRI-SAT/99. THE LETTER WAS FROM AN AVIATION SAFETY INSPECTOR. HE ADVISED ME THAT HE HAD INSPECTED ON ARR THE ACFT THAT I COMMANDED FOR FLT RETURNING TO DFW FROM KIX ON SEP/XX/99. THE INSPECTION REVEALED THAT THE COCKPIT DOOR LOCK HAD FAILED AND HE WAS UNABLE TO SECURE THE COCKPIT DOOR. HE NOTED NO ENTRY CONCERNING THE DOOR HAD BEEN MADE IN THE LOGBOOK. DURING THE FLT I RETURNED TO THE COCKPIT AT LEAST 6 TIMES AND ON EACH ENTRY I NEEDED TO USE MY COCKPIT DOOR KEY TO GAIN ENTRY. MY LAST ENTRY WAS APPROX 45 MINS PRIOR TO LNDG AND AT THAT TIME THE COCKPIT DOOR LOCK STILL REQUIRED A KEY FOR ENTRY. WHEN I SIGNED THE LOGBOOK UPON ARR, I BELIEVED THAT THE COCKPIT DOOR LOCK WAS OPERATIVE. NO FLC MEMBER ADVISED ME DURING THE FLT OR AT ITS CONCLUSION THAT THEY HAD EXPERIENCED ANY MALFUNCTION OF THE COCKPIT DOOR LOCK.
