Narrative:

On nov/xa/99 an 'a' check was performed on aircraft xyz. Main tires #6, #7, and #8 needed to be replaced for wear. I was involved in replacing the 3 wheel and tire assemblies. The aircraft was taxied to the gate. When on the gate, it was discovered that the #7 tire was flat. On investigation, the terminal mechanics found that the inner axle spacers were missing from tires #7 and #8. Upon hearing of the incident, the old tires were checked and the spacers were found stuck to the inner bearings.

Title: A B767-300 WAS RELEASED FOR SVC WITH #7 AND #8 WHEEL AND TIRE ASSEMBLIES REPLACED BUT OMITTING THE AXLE SPACERS.

Narrative: ON NOV/XA/99 AN 'A' CHK WAS PERFORMED ON ACFT XYZ. MAIN TIRES #6, #7, AND #8 NEEDED TO BE REPLACED FOR WEAR. I WAS INVOLVED IN REPLACING THE 3 WHEEL AND TIRE ASSEMBLIES. THE ACFT WAS TAXIED TO THE GATE. WHEN ON THE GATE, IT WAS DISCOVERED THAT THE #7 TIRE WAS FLAT. ON INVESTIGATION, THE TERMINAL MECHS FOUND THAT THE INNER AXLE SPACERS WERE MISSING FROM TIRES #7 AND #8. UPON HEARING OF THE INCIDENT, THE OLD TIRES WERE CHKED AND THE SPACERS WERE FOUND STUCK TO THE INNER BEARINGS.
