Narrative:

This incident occurred on dec/sat/97, and dec/sun/97. 3 mechanics (including myself) and 1 inspector were assigned to replace the first and second stage fan on air carrier aircraft xyz (B727) #1 engine (JT8) on the ramp at ZZZ. The engine subsequently was removed for overhaul this month after 3200 cycles. During overhaul it was discovered that a retainer was not installed on the first stage fan. The process of installing and securing the fan takes 2 mechanics to torque the tie rod nuts and witnessed by an inspector. I signed the items for installing the fan. Therefore, I take full responsibility, assuming no maintenance was performed within that time.

Title: A B727-200 WAS DISPATCHED AND OPERATED IN NON COMPLIANCE WITH THE #1 ENG MISSING A FIRST STAGE FAN BLADE RETAINER.

Narrative: THIS INCIDENT OCCURRED ON DEC/SAT/97, AND DEC/SUN/97. 3 MECHS (INCLUDING MYSELF) AND 1 INSPECTOR WERE ASSIGNED TO REPLACE THE FIRST AND SECOND STAGE FAN ON ACR ACFT XYZ (B727) #1 ENG (JT8) ON THE RAMP AT ZZZ. THE ENG SUBSEQUENTLY WAS REMOVED FOR OVERHAUL THIS MONTH AFTER 3200 CYCLES. DURING OVERHAUL IT WAS DISCOVERED THAT A RETAINER WAS NOT INSTALLED ON THE FIRST STAGE FAN. THE PROCESS OF INSTALLING AND SECURING THE FAN TAKES 2 MECHS TO TORQUE THE TIE ROD NUTS AND WITNESSED BY AN INSPECTOR. I SIGNED THE ITEMS FOR INSTALLING THE FAN. THEREFORE, I TAKE FULL RESPONSIBILITY, ASSUMING NO MAINT WAS PERFORMED WITHIN THAT TIME.
